Haven Homes Health Care Center earned a deficiency-free survey from the Minnesota Department of Health (MDH) in summer 2026, marking the second time in three years the community achieved this distinction. The results demonstrate compliance with federal and state standards for resident care, safety, and quality.
What is a Deficiency-Free Nursing Home Survey?
The survey followed a comprehensive review of the care and services provided at Haven Homes in Maple Plain, MN.
Nursing homes must comply with federal Centers for Medicare and Medicaid Services (CMS) requirements to receive payment under Medicare or Medicaid programs. The state completes regular, unannounced surveys to ensure settings remain in compliance with regulations and provide safe, quality care.
MDH surveyors evaluated the processes, procedures, general operations and physical plants, as well as information submitted by Haven Homes staff. Surveyors also interviewed staff, residents and family members of residents to gain perspective on the care provided at Haven Homes.
A deficiency-free survey indicates that surveyors found the community met applicable federal and state requirements at the time of the review.

According to Administrator Josh Brown, the achievement reflects a shared commitment across departments.

Two deficiency-free surveys in three years is something we are incredibly proud to celebrate.”
Brown says the recognition also serves as motivation to continue improving.
“While we’re honored by this achievement, we view it as motivation to continue raising the bar,” Josh says. “Our focus remains on continuous improvement, exceptional resident care, and maintaining the highest standards for those we serve and care for every day.”
